Job summary

CVS Health is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to deliver high-quality, evidence-based patient care in the community. You will assess needs, administer treatments, and collaborate with interdisciplinary teams.

You will document care in electronic health records and communicate effectively with patients, families, and colleagues while maintaining a clean, organized clinic environment and providing compassionate service.

Qualifications

• Active and in good standing LVN/LPN license.
• NCLEX-PN completion required.
• Knowledge of medical terminology and clinical documentation standards.
• Experience with phlebotomy, vaccine administration, and point-of-care testing.
• Excellent communication and organizational skills.
• Experience in a clinical setting with diverse cultural dynamics.
• Ability to handle sensitive information ethically and responsibly.
• Strong problem-solving and critical thinking skills.
• Proficiency with EMR systems; Epic experience preferred.

Responsibilities

• Administer oral, injectable, intravenous, and topical medications.
• Evaluate complex patient conditions, monitor pain levels, and document treatment responses.
• Monitor vital signs and record abnormalities in blood pressure, heart rate, respiratory rate, and temperature.
• Perform routine assessments to monitor patient health.
• Connect patients with community resources for medication administration and disease management.
• Conduct medication reviews to identify side effects and interactions.
• Develop and administer mental health assessments to screen for depression.
• Document patient medical history in the EMR.
• Interact with patients, families and healthcare professionals.
• Maintain a clean, organized clinic environment.
