The role emphasizes critical thinking, accurate documentation, and compassionate communication. Eligible candidates must hold an LPN/LVN license, NCLEX-PN, and be familiar with EMR systems such as Epic.

Qualifications

• Active and in good standing LVN/LPN license.
• NCLEX-PN licensed and eligible to practice.
• Strong knowledge of medical terminology and documentation standards.
• Experience with phlebotomy, vaccines and point-of-care testing.
• Excellent communication and organizational skills.
• Experience in a clinical setting with diverse patient populations.
• Ability to handle sensitive information ethically and professionally.
• Strong problem-solving, critical-thinking and decision-making abilities.
• Proficiency with EMR systems; Epic experience preferred.

Responsibilities

• Administer oral, injectable, intravenous, and topical medications.
• Evaluate patient conditions, monitor pain, and document responses.
• Monitor vital signs and record abnormalities.
• Perform routine health assessments to ensure well-being.
• Connect patients with community resources for medication and self-care.
• Conduct medication reviews for side effects and interactions.
• Develop and administer mental health assessments to screen for depression.
• Document patient history using electronic health records.
• Communicate effectively with patients, families, and healthcare staff.
