Job summary

CVS Health is seeking a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to deliver high‑quality patient care. You will assess needs, administer treatments, collaborate with teams and advocate for patients in community settings.

Your role includes med administration, vital sign monitoring, care coordination, and detailed EMR documentation to support positive outcomes. A strong clinical foundation and compassionate communication are required.

Qualifications

• Knowledge of medical terminology and clinical documentation standards.
• Experience with phlebotomy, vaccine administration, and point-of-care testing.
• Proficiency with electronic medical record (EMR) systems; Epic experience preferred.

Responsibilities

• Administer medications orally, via injection, IV and topical routes.
• Evaluate patient conditions, monitor pain, and document responses.
• Monitor vital signs and record abnormalities in blood pressure, heart rate, and temperature.
• Perform routine assessments to monitor health and well-being.
• Help patients access care and connect with community resources for medications and self-care.
• Conduct medication reviews for potential side effects and interactions.
• Develop and administer mental health assessments to screen for depression.
• Document medical history accurately in electronic health records.
• Communicate effectively with patients, families and healthcare professionals.
