Job summary

Qualifications

• Active and in good standing LVN/LPN license.
• Successful NCLEX-PN licensure.
• Knowledge of medical terminology and clinical documentation standards.
• Experience with phlebotomy, vaccine administration, and point-of-care testing.
• Excellent communication and organizational skills.
• Experience in a clinical setting with diverse cultures.
• Proficiency with EMR systems; Epic experience preferred.

Responsibilities

• Administer medications (oral, injectable, IV, topical) as prescribed.
• Assess patient conditions, monitor pain, and document responses.
• Monitor vital signs and record abnormalities.
• Perform routine assessments to track health and well-being.
• Coordinate patient access and connect to community resources.
• Review medications for interactions and side effects.
• Develop and administer mental health assessments as needed.
• Document patient history accurately in EMR.
• Communicate clearly with patients, families and healthcare staff.
