AuthoraCare Collective in Greensboro, NC seeks an LPN for home-based hospice care. This full-time role runs Monday through Friday, 8:00am–5:00pm, providing nursing services under the RN and coordinating patient care in the home setting.

You will administer medications, document care, and educate patients and caregivers while adhering to the Nursing Practice Act. Travel within the service area is required; on-call duties and weekend coverage may apply.

Qualifications

• Must be a graduate of an accredited LPN program.
• Must have a current North Carolina LPN license.
• One year's LPN experience necessary, preferably two years in an acute care, hospice, LTC or home health setting.
• Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
• Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

Responsabilités

• Delivery of Patient Care: Work within the Nursing Practice Act and provide treatments, documentation, patient and caregiver teaching, and administer medications as directed.
• Responds to emergencies and implements changes in nursing practice as needed.
• Conducts education and establishes therapeutic relationships with patients and caregivers.
