Job summary

AuthoraCare Collective in North Carolina is seeking a licensed practical nurse (LPN) for a full-time position in hospice care. The role involves delivering high-quality nursing care under the direction of a Registered Nurse, administering medications, and documenting care in the patient record.

You will collaborate with an interdisciplinary team to meet patient and family goals, provide teaching to caregivers, and participate in on-call coverage as scheduled.

Qualifications

• Graduate of an accredited LPN program.
• Current North Carolina LPN license.
• 1 year nursing experience; 2 years in acute care, hospice, LTC, or home health preferred.

Responsibilities

• Provide high-quality nursing services under RN supervision in home/hospice settings.
• Administer medications and document care per plan of care.
• Work with interdisciplinary team to meet patient/family goals.
• Provide weekend and week night coverage as assigned.
• Maintain timely and accurate patient records.

Education

LPN Diploma or Equivalent

NC LPN License

Hospice/LTC/Home Health Experience Preferred
