Job summary

3HC Home Health & Hospice Care Inc. is seeking a compassionate Hospice Nurse, LPN to join our Smithfield, NC team. This full-time role delivers skilled nursing care to hospice patients per individualized plans of care, under RN delegation, in a community setting.

You will collaborate with the IDG, document care in the EMR, and support families while promoting comfort and dignity. Ideal candidates are NC-licensed LPNs with hospice or acute care experience, CPR certified, reliable transportation,

Qualifications

• Licensed Practical Nurse currently licensed to practice nursing in the state of North Carolina.
• Minimum of two-years of experience as a nurse in an acute care setting preferred.
• Previous experience working with terminally ill patients preferred.
• Maintain current CPR certification (BCLS).
• Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public.
• Must be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patient's individualized plan of care.
• Possess valid Driver's License and reliable transportation.

Responsibilities

• The Hospice Nurse, LPN is responsible for administering skilled nursing care hospice patients requiring intermittent professional nursing services as outlined on the patient's individualized plan of care delegated by the RN.
• Creates positive experiences for internal and external customers that will meet their expectations.
• Assists with achieving positive patient outcomes and provides comfort to patient/family to facilitate quality of life and death with dignity.
• Assist with the on-going observation of the patient's physical and mental health and home environment by gathering and documenting data according to the plan of care.
• Documents in the medical record electronically and downloads patient information in EMR periodically throughout the day.
• Collaborates with supervisor to meet agency's clinical needs and rotates weekend, after hours, and holiday on-call with an RN on back up call.
• Actively participates in IDG meetings and reports updates from patient visits to the interdisciplinary team.
