Job summary

3HC Home Health and Hospice seeks an experienced Hospice Nurse (LPN) to join our Smithfield, NC team. You will provide skilled nursing care per the patient plan of care, administer treatments under RN supervision, and document care in the EMR daily.

This role requires NC LPN licensure and CPR certification, with prior hospice or acute care experience preferred. Must have a valid driver’s license and reliable transportation.

Qualifications

• Licensed Practical Nurse currently licensed to practice nursing in North Carolina.
• Minimum of two years of experience as a nurse in an acute care setting preferred.
• Previous hospice experience preferred.
• Maintain CPR certification (BCLS).

Responsibilities

• Provide skilled nursing care to hospice patients per individualized plan of care.
• Administer treatments and medications as delegated by the RN.
• Document patient care and updates in the EMR daily.
• Collaborate with the IDG and RN to ensure quality patient outcomes.
• Assist with on-call coverage and home visits as assigned.
• Maintain communication with supervisor and nursing staff to support continuous care.
