National Church Residences invites qualified LPNs to join our team providing compassionate nursing care and case management across multiple North Carolina counties. You will implement the plan of care, document thoroughly, and educate patients and families to support better health outcomes.

The role requires travel daily with a valid driver's license, and you will collaborate with RNs, physicians, and other clinicians while participating in on-call rotations and interdisciplinary meetings.

Qualifications

• Must hold an active LPN license in good standing.
• 12 months of nursing experience preferred, preferably in facility/home health.
• Must have valid driver's license and be able to travel daily.

Responsabilités

• Provide direct nursing care per plan of care.
• Document accurately and timely in patient records.
• Educate patients and families about diagnoses and care.
• Coordinate home health services with RN Case Manager and other disciplines.
• Participate in on-call rotation and team meetings.
