Job summary

Bethesda Health in Missouri seeks an experienced LPN to provide basic nursing care, assess residents on admission and ongoing basis, and communicate changes to the interdisciplinary team. You will document assessments, participate in care planning, and ensure safe medication administration.

Qualifications

• Licensed Practical Nurse (LPN) license required.
• CPR and AED certification required, or willing to obtain within six months of employment.
• IV certification required, or willing to obtain within six months of employment.
• Long-term care experience preferred.
• Must be able to assess basic nursing care needs of geriatric residents.

Responsibilities

• Provides basic nursing care and assesses residents on admission and ongoing basis.
• Communicates changes to IDT and family; participates in care planning.
• Documents assessments accurately; updates care plan as interventions change.
• Delivers nursing interventions including medication administration; IV only if IV certified.
• Notifies physician using SBARR; maintains composure in crises.
• Completes minimum twelve hours of continuing nursing education annually.
• May be assigned other duties as appropriate.
