Qualifications

• Education from an accredited nursing program leading to LPN licensure.
• Current LPN license in the state of employment.
• IV Certification required or obtainable within six months of employment.

Responsibilities

• Provide basic nursing care and ongoing resident assessments.
• Communicate changes in resident condition to IDT and families.
• Document assessments accurately; participate in care planning and updates.
