Qualifications

• Current state license as an LPN and CPR certification.
• Graduate of an approved LPN program (per state requirements).
• Minimum of 1 to 2 years of experience working as a nurse preferred.
• Previous experience with the elderly in paid or volunteer positions is highly desirable.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports significant changes in resident health to appropriate leadership.
• Directs and supervises daily work of Resident Care Assistants and mentors Lead RCAs.
• Coordinates care needs with community providers via case management.
• Balances service demands with supporting resident independence.
• Recognizes and reports elder abuse or neglect per state regulations.
• Responds to emergencies and uses personal emergency response system when needed.
