Job summary

Harmony Senior Services in Delaware seeks an LPN to provide quality nursing care and coordinate resident needs in an assisted living setting. You will work within a team, ensure documentation accuracy, and participate in meals delivery and safety protocols.

Qualifications

• Current LPN license to practice nursing in the state where the community is located.
• Experience working with the senior population, preferably in assisted living or LTC.
• Must be a team player who works well with others and has experience supervising staff.
• Must possess critical thinking skills, ability to effectively communicate verbally and in writing.
• Demonstrated ability to manage time efficiently with strong organizational skills.

Responsibilities

• Coordinate physical, social, and emotional care per physician orders and resident choices.
• Execute medication management plan with documentation and communication to physician as needed.
• Assess resident needs and update care plans as condition changes.
• Maintain safety and wellness measures with proper documentation.
• Assist with meals and monitor diets for adequate nutrition.
• Provide shift reports and motivation/direction to CNA/RMA staff.
• Respond to emergencies promptly and calmly.
• Establish positive working relationships with all departments.
