Job summary

Harmony Senior Services, LLC in Hockessin, Delaware is seeking a dedicated LPN for full-time evenings (3pm-11pm) to provide quality nursing care to residents in a respectful, dignified manner.

You will coordinate care with the medical team, manage medications, monitor changes in condition, assist with meals, document thoroughly, and contribute to a positive, safe living environment. Must be licensed in Delaware and committed to teamwork.

Qualifications

• Current LPN license to practice in the state of employment.
• Experience with senior population, preferably in assisted living or LTC.
• Team player with ability to supervise staff.
• Strong critical thinking and communication skills.
• Good time management and organizational abilities.
• Physically able to sit, stand, bend, kneel, walk and lift up to 50 lbs.

Responsibilities

• Provide quality nursing care and services to residents.
• Coordinate care with physicians and nursing team per orders.
• Execute medication management plan and document changes.
• Continuously assess resident needs and document condition changes.
• Assist with meals and monitor nutrition and diet acceptance.
• Maintain accurate records and communicate with Healthcare Director and shifts.
