Harmony Senior Services in Delaware is seeking a licensed practical nurse (LPN) to provide quality nursing care for residents in assisted living settings. You will coordinate physical, social, and emotional aspects of care, follow physician orders, and honor resident choices.

The role requires an active LPN license, experience with seniors, teamwork, strong communication, and good time management. You must be able to lift up to 50 lbs and work a flexible schedule including weekends and holidays.

Qualifications

• Current LPN license to practice nursing in the state where the community is located.
• Experience working with the senior population, preferably in assisted living or LTC.
• Must be a team player who works well with others and supervises staff.
• Must possess critical thinking skills and communicate verbally and in writing.
• Strong time management and organizational skills.
• Must be able to lift up to 50 lbs and perform transfers.
• Willing to work an 8-16 hour flexible schedule, including weekends and holidays.
• Must attend required trainings, CEUs, and staff meetings.
• Must be certified or willing to take state-required certification.

Responsabilités

• Provide quality nursing care and services to residents in a respectful and dignified manner.
• Coordinate physical, social, and emotional aspects of assisted living care per physician orders and resident choices.
• Execute medication management plan with documentation review and physician communication as needed.
• Assess resident needs and update care plans accordingly.
• Maintain safety, documentation, and high standards of record keeping.
• Collaborate with all departments and participate in activities and meals delivery as required.
• Report incidents and changes in resident condition with proper documentation.
