Job summary

Monarch Healthcare Management is seeking a Licensed Practical Nurse to provide residents with high-quality direct routine care under the supervision of the RN and in accordance with Minnesota BON standards and the Resident’s Bill of Rights.

The LPN participates in the development and modification of the resident Plan of Care, implementing delegated interventions, and monitors health data while reporting changes to the RN to support residents’ well-being.

Qualifications

• Current Minnesota License as a Practical Nurse.
• CPR certification and emergency care.
• Experience in skilled nursing and/or assisted living facilities.

Responsibilities

• Provide residents with direct routine care underRN supervision and in accordance with Minnesota BON standards and the Resident’s Bill of Rights.
• Participate in developing and modifying the Plan of Care and implement delegated interventions.
• Collect resident health data, monitor changes, and report to the RN; supervise unlicensed staff as needed.
