Monarch Healthcare Management seeks an LPN to provide high-quality direct routine care to residents under the supervision of an RN, adhering to Minnesota BON standards and residents' rights. The LPN gathers health data and reports responses while supporting the care team.

Responsibilities include participating in Plan of Care development, implementing delegated interventions, and guiding unlicensed staff to ensure safe, compassionate care that enhances residents' well-being.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota LPN license.
• CPR certification.
• Experience in skilled nursing or assisted living facilities.

Responsibilities

• Provide residents with direct routine care under RN supervision and in compliance with Minnesota BON standards.
• Participate in development and modification of the resident Plan of Care and implement delegated interventions.
• Collect resident health data and report changes to the RN.
• Assign and monitor nursing tasks to unlicensed staff after assessing their capabilities.
• Support residents' physical, mental and psychosocial well-being.
