Job summary

Monarch Healthcare Management is seeking an LPN to provide residents with high- quality direct routine care under the supervision of an RN, in accordance with Minnesota BON standards and the resident's Plan of Care.

You will collect health data, report changes to the RN, supervise tasks of unlicensed staff, and contribute to the facility's QAPI program while supporting residents' physical, mental, and psychosocial well-being.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota License as a Practical Nurse.
• Current CPR certification.
• Experience in skilled nursing and/or assisted living facilities.

Responsibilities

• Provide residents with direct routine care under the supervision of the RN and in accordance with the Minnesota BON standards and the Resident’s Plan of Care.
• Collect resident health data and report changes to the RN.
• Assign and monitor nursing tasks or activities of unlicensed staff and participate in QAPI initiatives.
