Wellspring Skilled Nursing & Rehab – Frankenmuth | Full-time, Part-Time, Per Diem | Hourly $30.40-$32.90

What Your Impact Will Look Like

You’ll provide hands‑on nursing care that supports residents physically, emotionally, and socially. You'll spend time getting to know residents, understanding their unique needs, and helping ensure their care reflects their personal preferences and goals. You'll serve as an advocate for residents while collaborating with physicians, nursing assistants, therapists, and other members of the interdisciplinary team to provide coordinated, person‑centered care. You'll monitor changes in condition, communicate concerns, implement physician orders, and help ensure residents receive timely interventions that support positive outcomes. You'll help guide and support nursing assistants during your shift while fostering teamwork and a shared commitment to resident care. You'll contribute to admissions, discharges, care planning, and ongoing assessments while helping create a safe and welcoming environment for residents and families. Some days you'll be providing comfort to a resident who needs reassurance. Other days you'll be supporting a family through important healthcare decisions, coordinating care with physicians, or identifying a subtle change in condition that prevents a larger health issue. Every day, you'll make a meaningful difference.

What You'll Bring

• Current Michigan Licensed Practical Nurse (LPN) license.
• Graduation from an accredited Practical Nursing program.
• Current CPR certification or willingness to obtain CPR certification.
• Nursing experience in skilled nursing, home health, medical-surgical, critical care, or related settings preferred.
• Strong clinical, assessment, communication, and documentation skills.
• Commitment to compassionate, resident-centered care.

Schedule

• Full-time, Part-time, Per diem
• Night shift, 12 hour shifts
• Rotating weekends and holidays required.
• Active involvement with residents, families, physicians, and interdisciplinary team members throughout each shift.
