Job summary

PruittHealth is hiring a Licensed Practical Nurse for the Georgia Blue Ridge area to conduct initial and reassessment visits in clients’ homes as part of the elderly and disabled waiver program. The role emphasizes patient care, medication review, documentation in Harmony, and coordination with the Home First team to ensure services support community living.

The position is full-time, 1st shift, with mandatory in-services and adherence to safety and compliance policies.

Qualifications

• Must be computer literate and able to use Harmony system.
• Must complete tasks within timelines set by DCH.
• Must communicate with translators for non-English-speaking clients/members.
• Keeps supervisor informed of daily activities and issues.
• Demonstrates teamwork and professional conduct with staff and the public.
• Adheres to company policies, laws, and safety regulations.

Responsibilities

• Completes initial and annual assessments in client homes and determines ability to complete IADLs and ADLs.
• Completes modified assessments for transfers, hospitalizations, and nursing home discharges.
• Assures services to help members remain in the community and meets fiduciary responsibilities.
• Communicates monthly with administrator to receive re-assessment schedules.
• Reviews medications for appropriateness and notes any changes.
• Documents assessment entries in Harmony per guidelines and requests records as needed.
• Participates in weekly Medical Director team meetings to review admissions, reassessments, hospitalizations, and discharges.
• Attends mandatory in-services and complies with safety and confidentiality requirements.
