Job summary

Home-Health-Services-of-Virginia is seeking Direct Support Professionals to provide in-home residential services to individuals with intellectual or developmental disabilities. You will work one-on-one in clients' homes to help them reach personal goals in the least restrictive setting.

Duties include assisting with budgeting, ADL/IADL tasks, community access, daily notes, quarterly reports, and ISPs. A valid driver's license and background checks are required; CPR/First Aid may be needed.

Qualifications

• High school diploma at minimum or equivalent course work
• Some PCA/CNA care-giving experience
• One year experience caring for individuals with developmental or physical disabilities
• A valid driver's license
• Current and valid auto insurance
• Current negative two part PPD test
• CPR/First Aid
• Medicaid Waiver Competency
• Eligible Criminal Background Check
• Eligible Central Registry Check

Responsibilities

• Assist individuals with budgeting and money management.
• Guide individuals with completing hygienic and household tasks.
• Engage individuals in social activities.
• Support individual with ADL/IADL completion.
• Support individuals with community access.
• Ensure individual health and safety is maintained.
• Complete daily progress note describing the progress of the individual each day.
• Complete quarterly reports describing individual progress.
• Follow outcomes and goals on Individualized Service Plans (ISPs).
• Attend in-service training sessions and workshops for professional development.
• Attend biweekly supervision sessions documenting individual progress.
• Maintain client confidentiality in all settings.
• Facilitate conflict resolution as needed.
• Attend physician appointments, SIS assessments, and Annuals related to individual goals.
