Job summary

Perfect Care INC in New Haven, CT is seeking a Recovery Assistant to support participants in their homes under the DMHAS waiver program. The role involves helping with daily living activities, meal planning, and coordinating with care teams to maintain safety and independence.

You will travel to participants’ homes, document care provided, and follow the Recovery Plan. A valid CT driver’s license, high school diploma, and Recovery Assistant training are required.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED required.
• Valid CT driver's license or state ID and DMHAS Recovery Assistant training proof; background check within six months.
• Commitment to ongoing training and education per DMHAS.

Responsibilities

• Provide Recovery Assistant services of at least 15 minutes in home and community settings.
• Assist with meal planning, shopping, housekeeping, dressing, hygiene.
• Support engagement in family, social, and recreational activities.
• Encourage use of medications as prescribed and healthy lifestyle habits.
• Participate in Recovery Plan development and quarterly updates as requested by DMHAS.

Education
