Perfect Care Inc. in New Haven, CT seeks a Recovery Assistant to provide home care services to participants in their homes and in community settings.

Hours are varied according to the Recovery Plan. Duties include meal planning and prep, shopping, housekeeping, dressing and personal grooming, facilitating medical and social activities, encouraging independence, and accompanying participants to appointments.

Qualifications

• Be at least 18 yrs old.
• Possess at least a high school diploma or GED.
• Possess a valid Connecticut driver's license or state issued identification card and be registered with the DMHAS as having completed an approved Recovery Assistant training program, have a Criminal Background check conducted within six months of application and meet any continuing education and/or training requirements set by DMHAS.

Responsabilités

• Recovery Assistant services of at least 15 minutes duration provided to the participant in his/her home and in community settings.
• Assisting or cueing the participant to perform or become engaged in family, social, and recreational activities.
• Assisting or cueing the participant to avoid risky behaviors, substance abuse, overspending, and unnecessary conflicts.
• Establishing and maintaining a supportive companionship relationship with the participant including escorting to medical appointments and reading or discussing activities.
• Participation in waiver Recovery Plan development and quarterly Recovery Plan update meetings, if requested by the DMHAS Support Coordinator.
• Travel with a participant when the Recovery Assistant is engaged in a qualifying waiver service activity.
