Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for plan members. Your visits last about 45 minutes and focus on health history, med review, and physical assessment using an iPad workflow.

You’ll connect patients with the right care, travel locally as needed, and provide services without altering current treatment unless directed by the plan.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required for NP/PA role.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations via iPad workflow.
• Connect with patients and bridge gaps in care.
• Travel locally and within licensed states as needed.
• Conduct virtual visits when required.
• Deliver Diagnostic & Preventive Services as indicated.
