Job summary

Signify Health, part of CVS Health, is hiring a Full-Time Clinician (NP or PA) to perform in-home health evaluations for plan members in their own homes. The visits last about 45 minutes, including medication and medical history reviews, a physical examination, and optional plan-ordered tests.

You’ll use an iPad with a streamlined clinical workflow, travel locally across licensed states, and contribute to bridging gaps in care without prescribing or ordering labs.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required for NP/PA role.
• Multi-state licenses preferred for cross-state visits.

Responsibilities

• Conduct in-home health evaluations for plan members.
• Review medications and medical history; perform physical evaluations.
• Provide diagnostic testing when ordered by health plan; use iPad-based workflow.
• Travel locally across licensed states to complete visits.
• Conduct virtual visits as needed.
