Job summary

Signify Health, part of CVS Health, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations across licensed states. You will use an iPad and follow a clinical workflow to review medical history, perform a brief physical, and connect members with the right care at their doorstep.

This role offers a part-time schedule (~29.5 hours/week) with benefits eligibility and a competitive pay range ($37.66–$81.11/hour).

Qualifications

• Active unrestricted license in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations for members.
• Engage with members and review medical history and medications.
• Coordinate with health plans and provide requested services.
