Job summary

Signify Health, part of CVS Health®, seeks a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. Visits last about 45 minutes and include history review, physical exam, and diagnostic tests as ordered by the member's health plan.

You will join a national network of clinicians, with travel to members' homes across licensed states and occasional virtual visits. Comprehensive benefits are provided for full-time staff.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations for health plan members.
• Engage with patients in their homes and address health questions.
• Travel locally and within licensed states; perform virtual visits when needed.
