Job summary

Signify Health, part of CVS Health®, seeks Per Diem Clinician (NP or PA) to provide in-home health evaluations for members in their own homes, bridging gaps in care.

The visit lasts about 45 minutes and includes med history review and physical assessment, using an intuitive iPad workflow. You’ll travel locally within licensed states, may conduct virtual visits, and join a national network of purpose-driven clinicians.

Qualifications

• Active, unrestricted license in WA (coverage area).
• Board certification required.
• Multi-state licenses preferred.
• Experience using iPad-based clinical workflows is a plus.

Responsibilities

• Provide in-home health evaluations for health plan members.
• Travel locally within licensed states to member homes.
• Engage with members to bridge gaps in care and educate on health plans.
