Job summary

Signify Health, part of CVS Health, seeks an in-home clinician (NP or PA) for a part-time, per diem role. You will perform 45-minute evaluations at members' homes, reviewing meds and history, with optional in-home diagnostics.

Travel locally and across licensed states is expected, and virtual visits are possible. We value compassionate patient engagement and accurate documentation using an iPad-based workflow.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Provide in-home health evaluations to members in their homes.
• Review medication history and perform physical evaluations.
• Conduct additional diagnostic tests as ordered by health plans.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Utilize Signify Health workflow on an iPad and adhere to clinical guidelines.
