Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. Visits include medication/history review and physical exams, with diagnostic tests ordered as indicated by the plan.

You will use an iPad and follow a clinical workflow; no prescriptions or lab orders are required by the clinician. The role is part-time, per diem, and supports travel to homes within licensed states.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification is required for NP/PA roles.
• Multi-state licenses are preferred to enable travel across states.

Responsibilities

• Provide in-home health evaluations for assigned patients.
• Review medications and medical history, perform physical evaluations.
• Order diagnostic tests when indicated by the health plan.
