LifeCare Home Health and Hospice Family is seeking compassionate Home Health Aides to support patients in their own homes. You will monitor patient condition, assist with ADLs, and help maintain comfort and independence while following the Plan of Care and agency policies.

You will collaborate with the care team to ensure high-quality in-home care. Ideal candidates have a HS diploma, HHA or CNA certification, CPR, a valid driver’s license and transportation, and at least one year of relevant

Qualifications

• High School graduation or equivalent.
• Home Health Aide Certificate or similar certification from an accredited training course.
• Minimum of one year experience as a Home Health Aide or CNA.
• Strong English language both written and verbal.
• Active CPR certification required; Valid driver’s license and reliable transportation required.

Responsabilités

• Monitors and records patient condition as part of the Plan of Care (POC).
• Provides personal care, ADL assistance, and housekeeping tasks.
• Teaches families how to assist with lifting, turning, and repositioning the patient.
• Interacts with family members to address concerns and educate on treatment and aftercare needs.
• Maintains patient information in the EMR and notifies leadership of changes.
