Job summary

LifeCare Home Health & Hospice Family is seeking compassionate Home Health Aides to deliver high-quality care in patients' homes. Aides support daily living activities, monitor health, and promote independence while upholding the agency's mission and standards.

Responsibilities include patient assessments, collaboration with the care team, and educating families on treatment and aftercare. Applicants should have a HHA certificate, CPR, and at least 1 year of experience.

Qualifications

• High School graduate or equivalent.
• Home Health Aide Certificate or similar certification in good standing.
• Minimum of 1 year experience as a home health aide or CNA.
• Strong English language skills (written and verbal).
• Active CPR certification required.

Responsibilities

• Monitor and record patient condition and plan of care with the care team.
• Assist with activities of daily living and personal care.
• Provide housekeeping and meal preparation support as needed.
• Assist with grooming, bathing, dressing, and mobility.
• Educate families on patient care and safety procedures.
• Maintain patient information in the EMR and communicate changes to leadership.
