Job summary

The Action Health Staffing & Home Care Services team seeks a Certified Nursing Assistant to provide in-home care to clients in the Williamston area. This role covers daytime and early evening hours, working closely with supervising clinicians and adhering to the Plan of Care.

Responsibilities include direct patient care, personal hygiene support, transfers, basic nursing tasks, meal prep, companionship, and documenting activities using EVV. Strong communication and reliability are essential.

Qualifications

• High school diploma or GED preferred.
• Effective communication and ability to follow directions.
• Must be physically capable of lifting and assisting clients.

Responsibilities

• Provide direct patient care as assigned in the home setting.
• Assist with personal hygiene, transfers, and ambulation.
• Provide basic nursing care and prescribed exercises as directed.
• Prepare meals per client preferences and special diets.
• Provide companionship and light housekeeping as needed.
• Maintain accurate records and report changes in condition.
• Adhere to the care plan and EVV/task time sheets.
• Uphold patient confidentiality and comply with Plan of Care.
