Qualifications

• Master of Social Work required from an accredited School of Social Work.
• Minimum 2 years of relevant experience in care management or mental health.
• Knowledge of social determinants of health and culturally responsive practice.
• Ability to triage needs and collaborate with an interdisciplinary team.
• Experience with electronic medical records and basic computer applications.

Responsibilities

• Float to various sites within Legacy Health System to provide care management social work.
• Conduct psychosocial assessments and coordinate with patients, families and teams.
• Support patients with transitions, crisis management and resource referrals.
• Apply strengths-based approaches to enhance patient and family capacity.
