Qualifications

• Master's Degree in Social Work is required.
• WA/TX: Preferred - 1 year of post-Master’s experience in a health care setting.
• AK: Preferred - 2 years of supervised, post-Master’s experience in a psychiatric or other health care setting.
• Required: BLS (American Heart Association, Vendor Managed) for Home Health.
• Preferred: Social Worker License upon hire.

Responsibilities

• Assess patients’ psychosocial, financial, environmental, and community needs.
• Provide direct and indirect social services for patients and families in homes, long-term care facilities, and hospitals.
• Deliver crisis intervention and supportive counseling.
• Complete documentation to ensure regulatory compliance.
• Act as a member of the interdisciplinary team and support care objectives.
• Attend team meetings and contribute to care planning.
