Job summary

Legacy Home Health Agency, Inc. is seeking a dedicated Social Worker to provide coordinated in-home care, perform assessments, counseling, and referrals to meet patient and family needs.

The role requires a Master's in Social Work, licensure (LCSW or LMSW), at least one year in a healthcare setting, and strong knowledge of community resources. Regular travel across the Rio Grande Valley is expected, with mileage reimbursement provided.

Qualifications

Responsibilities

The primary function of the Social Worker is to provide coordinated care in the home to patients and perform assessments, analysis, counseling and referrals to meet the needs of the patient and their family.

Essential Functions:

• Assess patients to identify needs.

• Assist other team members in understanding the social and emotional factors related to the health problems.
  
• Utilize and provide patient/family with appropriate resources as needed.

This position requires occasional travel to our Corpus Christi region, as well as regular travel throughout the Rio Grande Valley for visits. Our service area ranges from Brownsville to Rio Grande City!

Mileage reimbursement is provided.

Job Qualifications:

• Education: Masters Degree in Social Work
• Licensure: LCSW or LMSW required*
