Qualifications

• Accredited Case Manager Certification (ACMA:ACM-SW) preferred.

Responsibilities

• Provide supportive counseling, end-of-life support, care planning and brief interventions.
• Document interventions and outcomes in electronic health records while maintaining confidentiality.
• Collaborate with the care team to identify coping strategies and connect patients with community resources.
