Job summary

Providence at Home with Compassus is seeking a compassionate Hospice Social Worker to support patients and families across homes, facilities, and hospitals. You will assess psychosocial, financial, and environmental needs, provide crisis intervention and end-of-life counseling, and coordinate with care teams to ensure compliant, patient-centered planning.

Required: Master's in Social Work and Washington licenses; preferred one year post-Master's experience in healthcare.

Qualifications

• Master's degree from an accredited School of Social Work is required.
• Preferred: 1 year post-Master's experience in health care setting.

Responsibilities

• Perform psychosocial assessments and document in Plan of Care and team notes.
• Deliver social services across homes, long-term care facilities, and hospitals.
• Provide crisis intervention and supportive counseling for terminal illness.
• Coordinate with interdisciplinary team for care planning and discharge planning.
