Job summary

MultiCare Capital Medical Center in Olympia, WA, seeks a Social Worker (MSW) to support patients and families facing personal and environmental challenges that affect home care. This 0.5 FTE, day shift position offers meaningful work and a path to growth within a trusted health system.

The role emphasizes assessment, data gathering, collaboration with families and community resources, and careful documentation of plans, with required Master’s in Social Work and licensure as applicable

Qualifications

• Master's degree in Social Work required.
• Hospital/acute care experience preferred.
• Experience related to the medical social work function.

Responsibilities

• Administer assessment tools and gather data for assessment, and to assess for optional needed services.
• Interview families to determine social work needs of the participant.
• Contact community resources to coordinate services.
• Provide initial and ongoing assessment of participant's social, emotional and cognitive status and needs, involving participant and other staff members in the assessment.
• Ensure accurate documentation is maintained for all assessments, treatment plans, and evaluations.
