Job summary

Three Oaks Hospice, Inc. is seeking a compassionate Social Worker to provide coordinated home-based care for patients of all ages, performing psychosocial assessments, counseling, and referrals to meet patient and family needs.

Responsibilities include completing the Comprehensive Assessment within five days of hospice election, assisting with advance directives and discharge planning, and participating in IDG/IDT meetings to coordinate care.

Qualifications

• Bachelor's or Master’s degree in social work from an accredited program.
• Current LMSW license.
• Knowledge of community resources and strong interpersonal skills.
• Current State Certification.

Responsibilities

• Provide coordinated care in the home for patients of all ages, including psychosocial assessments, counseling, and referrals.
• Complete Comprehensive Assessment within five days of hospice election to identify needs.
• Assist with advance directives, discharge planning, and family counseling.
• Participate in IDG/IDT meetings and coordinate patient care.
• Promote hospice philosophy and participate in QAPI and infection control programs.
