Job summary

Three Oaks Hospice is seeking a licensed social worker to provide coordinated care in patients’ homes across South Atlanta and surrounding areas. You will perform psychosocial assessments, counseling, and referrals to support patients and families.

The role includes completing assessments within five days of hospice election, participating in IDG/IDT meetings, assisting with advance directives and discharge planning, and promoting hospice philosophy while maintaining confidentiality.

Qualifications

• Master’s degree in social work from an accredited program.
• Current and valid LMSW license.
• Working knowledge of community resources; strong interpersonal skills.
• Reliable transportation and valid driver's license with auto liability insurance.
• State Certification or licensure as required for the role.

Responsibilities

• Provide coordinated care in patients' homes and perform psychosocial assessments.
• Identify patient and family needs and refer to community resources and supports.
• Assist with advance directives, discharge planning, and family counseling.
• Participate in IDG/IDT meetings and in the patient care planning process.
• Counsel patients and families on end-of-life, death, and grief issues.
• Contribute to quality improvement and uphold privacy and infection control policies.
