Qualifications

• Bachelor’s or Master’s degree in social work from a State accredited program.
• Current and valid LMSW license.
• Working knowledge of community resources; good interpersonal skills; Hepatitis profile.
• Current State Certification.
• Reliable transportation; Valid Driver’s License and current auto liability insurance

Responsibilities

• Provides coordinated care in patients' homes across all ages and performs psychosocial assessments, analysis, counseling, and referrals.
• Completes Comprehensive Assessment within five days of hospice election to identify psychosocial, financial, environmental, and community needs.
• Assists with advance directives, discharge planning, family counseling, and care-level changes as indicated.
• Maintains effective verbal and written interpersonal communication.
• Meets mandatory continuing education requirements of the Hospice and licensing board.
• Stays informed about public and private eligibility standards and requirements.
• Counsels patients and families on end-of-life, death, and grief.
• Promotes hospice philosophy and policies to ensure quality of care.
• Attends IDG/IDT meetings and coordinates care with the IDG/IDT.
• Accepts assignments aligned with education and competence to meet patient needs.
• Maintains patient privacy and a respectful environment.
• Participates in Hospice’s QAPI program and educational in-service programs.
