Responsibilities

• Provide hospice social work services per the individualized care plan.
• Coordinate all aspects of patient care in collaboration with the care team.
• Address emotional and biopsychosocial needs of patients and families at end of life.
• Collaborate with interdisciplinary team; advocate for patient/family across settings.
• Conduct crisis intervention and counseling as needed.
