Job summary

Hireology is seeking a compassionate Hospice Aide Scheduler to coordinate hospice aide visits and maintain schedules that meet patient needs and staff availability. You will thrive in a fast-paced setting, communicating with patients, families, and staff to ensure timely care.

Ideal candidates have experience in hospice, home health, healthcare scheduling, or staffing, plus strong organizational and communication skills.

Responsibilities

• Collaborate to coordinate and maintain daily and weekly schedules for Hospice Aides/CNAs.
• Ensure timely, consistent visits aligned with patient care plans.
• Match aides with patients based on location, availability, and needs.
• Communicate schedule changes with all stakeholders.
• Monitor missed or rescheduled visits and address coverage gaps promptly.
• Assist with filling open shifts and coverage when aides are unavailable.
• Maintain scheduling records and documentation accurately.
• Review upcoming schedules for potential staffing gaps.
• Coordinate with clinical team on changes in patient needs.
• Ensure schedules align with patient care plans and policies.
• Respond professionally to patient, family, and staff inquiries.
• Assist with onboarding and scheduling new Hospice Aides as needed.
• Maintain patient confidentiality and protect information.
• Support scheduling responsibilities as needed.
