Job summary

Chapters Health System is seeking a Per Diem RN for the Inpatient Unit to deliver compassionate hospice care, coordinating the Plan of Care with the Interdisciplinary Group and providing symptom relief to patients and families.

The RN must hold an active state license, BLS certification, and have at least one year of nursing experience. A valid driver’s license and reliable transportation are required to support visits and mobility.

Qualifications

• Current RN license for the state of practice.
• Active BLS certification.
• Minimum 1 year of nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and reliable transportation.
• Effective English communication (verbal and written).
• Flexible schedule to meet patient needs.

Responsibilities

• Provide direct care to patients and families as part of the IDG, incorporating psychosocial, spiritual, cultural and physical components.
• Coordinate the Plan of Care reflecting patient’s evolving needs.
• Educate patient and family on disease process, palliative care and safety practices.
• Report changes in patient condition to IDG or physicians.
• Obtain physician orders and ensure timely documentation.
• Assist with admissions, care planning and discharge processes.
• Support bereavement and communication with care teams as needed.
