Job summary

Affinis Hospice in Georgia is seeking a dedicated Registered Nurse to provide professional nursing care to Hospice Services patients. You will identify patient and family needs and deliver supportive care per physician orders and the hospice plan of care.

As part of the interdisciplinary team, you will assess patients within 48 hours of admission, coordinate with physicians and staff, maintain accurate records, and participate in IDG conferences.

Qualifications

• Graduate from an accredited School of Nursing.
• Must be licensed in GA as a Registered Professional Nurse.
• Minimum six months acute care RN experience; over one year preferred; home care or hospice experience preferred.
• Current CPR.
• Ability to meet Hospice health clearance and provide professional references.

Responsibilities

• Provide professional nursing care to Hospice Services patients as ordered by the attending physician.
• Assess the patient within 48 hours of admission and develop the care plan with the IDG.
• Coordinate care with physicians, Hospice staff, and other agencies to meet patient needs.
• Maintain up-to-date patient records with problems, plans, actions and goals.
• Lead interdisciplinary group discussions and participate in ongoing caseload management.
• Support patients and families through end-of-life care and bereavement needs; participate in on-call coverage.
