Job summary

Aachele Home Care and Hospice, LLC is seeking a compassionate Hospice Registered Nurse (RN) to join our team serving Stone Mountain, Georgia and surrounding areas. The RN will perform skilled hospice nursing visits in patients' homes, manage symptoms, educate families, and collaborate with the interdisciplinary team.

The ideal candidate has a Georgia RN license, CPR/BLS, and at least one year of nursing experience, with hospice or home health experience strongly preferred.

Qualifications

• Current, unrestricted Georgia RN license.
• Graduate of an accredited nursing program.
• Current CPR/BLS certification as required.
• Minimum 1 year of professional nursing experience preferred.
• Hospice/home health/palliative care experience strongly preferred.
• Ability to work independently in a home-based environment.

Responsibilities

• Perform skilled hospice nursing visits in the patient's home or residential setting.
• Complete comprehensive and ongoing nursing assessments.
• Assess pain, symptoms, functional status, safety concerns, and changes in condition.
• Provide comfort-focused nursing care and symptom-management interventions.
• Administer and manage medications and treatments per orders and plan of care.
• Educate patients and caregivers regarding medications, disease progression, comfort measures, safety.
• Develop, implement, review, and update nursing plan of care.
• Communicate changes to the hospice team and provider promptly.
• Participate in team meetings and care-plan reviews.
• Coordinate patient care with physicians, social workers, chaplains, aides, and others.
• Provide supervision to hospice aides and ensure care aligns with plan.
• Complete nursing documentation accurately and timely.
• Assist with admissions, recertifications, transfers, discharges, and other processes.
• Provide patient/family education regarding hospice services and end-of-life care.
• Respond to changes in condition and symptom-management crises.
• Follow infection-control, HIPAA, and agency policies.
• Participate in orientation, ongoing education, and quality-improvement activities.
• Provide on-call or after-hours coverage when scheduled.
