Job summary

Agape Hospice Care in Georgia seeks a registered nurse to administer skilled nursing care in patients' residences, coordinate with the care team, and guide families through end-of-life care.

The role requires an associate or bachelor’s degree in nursing, state RN license, CPR, and at least two years of clinical experience; hospice or home health experience preferred. Proficiency with computers and MS Office is expected, along with strong communication and organizational skills.

Qualifications

• RN license in good standing with the state.
• Two years’ RN experience in clinical care; hospice/home health preferred.
• Current CPR certification.
• Proficient computer and software use; MS Office familiarity.
• Strong interpersonal and communication abilities.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients' needs.
• Implement the individualized plan of care and adjust as needed.
• Educate patients and families about disease, self-care, end-of-life care.
• Initiate appropriate nursing procedures as indicated.
• Prepare progress notes showing goal attainment.
• Coordinate services with the inter-disciplinary team and referrals.
• Communicate changes in patient needs to the physician and team.
• Determine scope and frequency of services based on acuity and needs.
• Assess caregiver ability to meet patient needs and provide support.
• Provide on-call services as assigned.
• Execute other duties as assigned.
