Job summary

Aachele Home Care and Hospice, LLC is seeking a qualified Home Care Registered Nurse (RN) to deliver in-home nursing care, assess patient needs, and implement individualized plans of care. The role emphasizes assessment, education, and coordination with physicians to ensure high-quality patient outcomes.

You will work within agency policies and federal/state requirements, documenting care and communicating changes to the clinical team.

Qualifications

• The RN provides skilled nursing services in home care settings.
• Assess and document patient conditions and plan of care.
• Coordinate with physicians and the healthcare team to implement orders.

Responsibilities

• Perform comprehensive and focused nursing assessments.
• Obtain and document vital signs and other clinical measurements.
• Assess pain, mobility, nutrition, hydration, skin integrity, safety, medication use, and functional status.
• Provide skilled nursing interventions according to physician/provider orders and the plan of care.
• Monitor the client’s response to treatment and identify changes in condition.
• Provide medication education and reconciliation as appropriate.
• Educate client and caregiver regarding services, medications, treatments, and emergency procedures.
• Coordinate necessary follow-up with the physician/provider and agency clinical team.
• Complete all required admission documentation accurately and timely.
