Job summary

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient with computer and Microsoft Office suite.

Responsibilities

• Administer skilled nursing care to clients in their homes.
• Coordinate care with the IDT and patient/family.
• Develop and update the plan of care based on assessment.
• Provide education to patient/family on disease, care, and end-of-life planning.
• Perform on-call duties and coordinate after-hours care as needed.
